JFSWNY is seeking a Health Home Specialist - Adults to work with our clients from the Ukrainian community.

WHAT YOU'LL DO

In working with your clients, you'll be promoting client rights and access to social services, social security, mental health, and other support services. As part of the Care Manager Team, you'll manage all aspects of care for Health Home consumers. These responsibilities include tracking and arranging appointments, working in partnership with treatment providers, offering consumer advocacy and education, and coordinating other aspects of the consumer's community services.

Equally important are keeping accurate documentation, maintaining case notes, and completing (and monitoring!) all required forms. For this reason, you must not only have proficient communication skills but must also have excellent writing skills and attention to detail.

WHAT YOU'LL BRING

  • Requires knowledge of the Ukrainian language
  • Experience in care coordination with community-based client services or discharge planning desirable
  • Ability to effectively provide case management services, responding supportively, and with persistence, to client families
  • Familiarity with community linkage resources helpful
  • Willingness to work within the community and make home visits
  • Experience with electronic medical records a plus
  • Proficient verbal & written communication skills for interactions and for documentation
  • Familiarity with and appreciation of issues related to people with diverse cultures and languages

This starting wage for this position is between $20 - $22.50(midrange) per hour dependent on experience and language. We offer great benefits including 4+ weeks accrued Paid Time Off (PTO), plus 13 paid holidays, excellent health insurance options, 401k, and much more. Additionally, we have transitioned to a 35 hour work week with an early close on Fridays!

We are conveniently located at the Tri-Main Building at 2495 Main Street. Since this position requires work in the community, you must have a reliable vehicle, a clean driver's license and required liability insurance levels of 100/300K. If hired, you must produce evidence of having received the Covid-19 vaccination and undergo a DOH background check.
